Give the Gift of a Lifetime
More than 87,000 Americans
are waiting for organ transplants...
2,800 in New Jersey
alone.
Organ transplants save thousands of lives
each year. Unfortunately, there remains a tremendous shortage of
donor organs because many people do not properly declare their
wishes to donate.
In an effort to increase donation, the
New Jersey Organ and Tissue Sharing Network has established an
Organ and Tissue Donor registry. By joining the Donor Registry
and telling your family about your wishes to become a donor, you
could save up to eight lives and enhance the lives of up to 75
others through tissue donation. The Donor Registry can help insure
that families are made aware of their loved one's wishes when there
is a possibility for organ and tissue donation.
Registration is Easy

Step
1: Simply print out and complete the Donor
Registry Form, have
it witnessed by two people, and return
it to The Sharing Network.
Step 2: Print
out, complete and sign the temporary donor
card. Be sure to obtain
the signatures of two witnesses. A permanent donor card
with your Sharing Network registration number will be sent within
10 weeks. Likewise, your designated next of kin will receive a
letter notifying them of your decision to donate. It is important
to discuss your decision to donate with your family.
Step 3: Carry
the donor card with you at all times. Your information will be
stored in a computer database
where it will be used, when appropriate, to inform your designated
next of kin and/or appropriate health care personnel of your desire
to donate. The Sharing Network may also gather statistical information
from this registry to assist in their educational outreach.
